[Alleghany, Ashe and Watauga Counties, NC] — AppHealthCare and your local hospital are seeking your input on the health needs of Alleghany, Ashe and Watauga Counties. Community members are invited to fill out the short Community Health Opinion Survey.
The survey is available in English and in Spanish at www.apphealthcare.com, all AppHealthCare locations, your local hospital, and other community locations. It is available in both electronic and paper form. The survey is anonymous and participants can enter to win a $100 gift card.
AppHealthCare will use the community health opinion survey results to address major health and community issues. Alleghany Memorial Hospital, Ashe Memorial Hospital, and Appalachian Regional Healthcare Services will also use the survey results to better meet the health needs of the community.
